Minnesota Medical Marijuana Program: A Guide For Patients

These are the steps you need to take if you're eligible for the Minnesota Medical Cannabis Program.

  • Long-lasting pain

  • Sickle cell disease could be described as a kind of sickle cell disease.

A physician assistant, doctor or advanced practice registered nurse must first confirm your health before you can apply for the program.


  • Make contact with your physician, physician assistant, or advanced practice registered nurse to schedule an appointment. They are the only medical professionals that are able to verify your medical health.

  • The OMC does not keep track of those who are participating in the program. It is your obligation to make an appointment with a registered health care professional.

  • The Patient Acknowledgement and Email Form is available for download and printing (PDF).

  • It is necessary to fill out the below areas on the Patient Email and Acknowledgement Form


  • Please provide the email address where you would like to receive secure messages about your health. Think about creating a brand new personal email account prior to the appointment if already have one at your current job.

  • Take this form with you to your appointment, and then hand it over to your doctor.

  • Get a written summary of your visit and a list of all medicines you're currently taking during your appointment. When you get your medical marijuana take this paperwork with you.


Following your visit with your physician Following your visit, you'll get an email from the OMC which contains the following information:

After you've received your confirmation email, then you will be able to register. Before you click on the Register button, collect the following information:


  • Examples of government-issued IDs include identification cards issued by the state, drivers' license, and passports.

  • If you are applying for the discount fee, then you must be able to prove that you're receiving Supplemental Security Income, Social Security Disability (including those who transition into retirement benefits) medical aid, MinnesotaCare or IHS, Railroad Disability, VA dependency, indemnity compensation or Veteran's Disability benefits.

  • To capture or scan images of your IDs use the camera on your phone or with cameras.

  • The annual registration fee may be paid with a credit/debit card or cash, and the program's payment processor, U.S. Bank, accepts all these methods of payment.

  • After your account is approved, you will get an email. You can then proceed to Step 3: Receive, which is the final step in the procedure.


It is necessary to first fill out an Assessment of the Patient through your Registry account before you are able to buy medical cannabis. This form must be filled out by the person permitted to pick up your medicinal cannabis if you are younger than 18 or if you are the assistance of a caregiver.

  • Each time you seek medical cannabis, you must submit a new patient Self-Evaluation.

  • Bring your appointment information and the list of medications you currently receive from your doctor that you obtained during your health physician visit at the time you visit a Cannabis Patient Center.


You'll visit with a qualified pharmacist at the Cannabis Patient Center, who will:


  • Review your self-evaluation report Summary of your appointment with your physician and the list of medicines you're now taking.
